The mission of Five County Mental Health Authority (FCMHA) is to assist individuals and families to be healthy, experience personal satisfaction and to live with dignity in their own community. FCMHA believes that unless a consumer has safe, decent and affordable housing, even the best treatment and rehabilitation services won’t be effective. Supportive housing allows adults with disabilities to experience a sense of independence and belongingness to their community. FCMHA is committed to developing a comprehensive program of supportive housing options for persons with mental illness, substance abuse and developmental disabilities in Vance, Granville, Franklin, Warren and Halifax counties.

Eligibility: Adult consumer of the Five County MH/DD/SAS service system and receives support services from a Five County network provider.
